Inschrijven

Onze praktijk is geopend voor nieuwe patiënten! Woont u in Oosterhout? Dan kunt u zich bij ons inschrijven. Bij ons krijgt u geen assistente aan de telefoon. We willen het u zo gemakkelijk mogelijk maken om direct contact met de dokter te hebben. Om u te kunnen inschrijven zijn een werkend 06-nummer, mailadres en gebruik van MijnGezondheid.net (met DigiD) noodzakelijk.

Hoe schrijft u zich in?

U schrijft zich eenvoudig in door het inschrijfformulier in te vullen.

Uitschrijven bij uw vorige huisarts

U moet u ook uitschrijven bij uw vorige huisarts. Dit doet u zelf. Vraag dan ook of uw vorige huisarts uw medisch dossier doorstuurt naar onze praktijk.

Vergeet ook niet om ons als nieuwe huisarts door te geven aan het ziekenhuis en andere gezondheidsinstellingen waar u bekend bent.

Registration

Our practice is open to new patients! Do you live in Oosterhout? If so, you can register with us. When you call our practice, you will not speak to a medical assistant. We want to make it as easy as possible for you to have direct contact with the doctor. To register with us, a working mobile phone number, an email address, and the use of MijnGezondheid.net (with DigiD) are required.

You can easily register by completing the registration form.

Deregistering from your previous GP
You must also deregister from your previous GP. You do this yourself. Please also ask your previous GP to transfer your medical records to our practice.

Don’t forget
Please remember to inform the hospital and any other healthcare institutions where you are registered that we are your new GP.
